Physician-Linked Correspondence
Primary Care Providers play a key role in the success of cancer screening programs. Evidence shows a positive relationship between physician recommendation for screening and patient participation in colorectal cancer, breast cancer and cervical screening. This link will take you to the Ontario Health website to register for physician linked correspondence.
Out-of-Hospital Premises (OHP) and Hospital Colonoscopy Sites

The following criteria can help determine if a referral is appropriate to send to an OHP. Do NOT send a referral to an OHP if your patient meets any of the following:
- BMI greater than 40
- asthma or COPD with admission to hospital in the last 12 months
- any diagnosis of congestive heart failure, ever
- any chest pain currently using nitroglycerin
- concerns with unaided ambulation, dressing, stretcher transfer
- renal dialysis
